Posted on Dec 20 2018

EverCare At Home, EverCare’s Certified Home Health Agency (CHHA), has recently been awarded “deemed status” and the seal of excellence by Community Health Accreditation Partner, Inc. (CHAP), the leading accrediting organization for the home health care industry.

CHAP Accreditation demonstrates that EverCare meets the industry’s highest nationally recognized standards. The rigorous evaluation by CHAP focuses on processes, structure and function, quality of clinical services and products, human and financial resources, and long-term viability. Simply stated, adherence to CHAP’s standards leads to better quality care.

“The CHAP Accreditation reinforces our commitment to providing the highest standards of quality in the deliverance of community-based care,” said EverCare President and CEO Sylvia McTigue. “As our team works hard on a good patient experience of care and improving the health of populations, we are pleased to be recognized by this leader in accreditation for the home health care industry.”

EverCare at Home’s CHAP Accreditation was awarded under the CHAP Home Health Standards of Excellence. By achieving CHAP Accreditation, EverCare has also been deemed to meet the Medicare Conditions of Participation and is certified as a Medicare provider.

“By achieving CHAP Accreditation, EverCare at Home has shown a commitment to excellence,” said CHAP President & CEO Karen Collishaw. “We are excited to begin our partnership with EverCare at Home by offering support in its commitment to providing quality care and continuous improvement.”

EverCare At Home provides patients and their families in the Hudson Valley with services including: physical, occupational, and speech therapy, nutritional evaluation and planning, skilled nursing, home health aides, social work, and medical supplies, equipment, and appliances. The Certified Home Health Agency brings quality support to patients in the comfort of their homes. Whether managing an existing condition, recovering from an injury or illness, or working to restore self-sufficiency, EverCare professionals work closely with patients to create an individualized care plan that meets their needs and addresses their wishes.

CHAP is an independent, not-for-profit, accrediting body for community-based health care organizations. Created in 1965, CHAP was the first to recognize the need and value for accreditation in community-based care. CHAP is the oldest national, community-based accrediting body with more than 9,000 agencies currently accredited nationwide. Through “deeming authority” granted by the Centers for Medicare and Medicaid Services (CMS), CHAP has the regulatory authority to survey agencies providing home health, hospice and home medical equipment services, to determine if they meet the Medicare Conditions of Participation and CMS Quality Standards. CHAP’s purpose is to define and advance the highest standards of community-based care.
