EverCare is a 501(c)(3) not-for-profit organization.

EverCare Choice is our Managed Long Term Care Plan.  We provide nearly 1,000 patients with access to care that allows them to remain in their homes for as long as safely possible, supported by strong clinical teams and a vast Provider Network of nearly 200 trusted and interconnected vendors.

EverCare at Home is our Certified Home Health Agency.  We are confident that your members will greatly benefit from the services we offer along with our Fast Track Joint Rehab Program, our hospital prevention initiatives and our strategic quality goals as a premier Certified Agency in the Hudson Valley.

EverCare Life is our Social Adult Day Care Services Program. We have been providing quality care for adults in our day care program for over 16 years. We are certified by the New York State Office for the Aging.
