UAS Assessment Nurse

Calling all flexible, travel-ready UAS Assessment Nurses with an eye for evaluation and the skillset for care planning! EverCare is currently seeking several UAS Assessment Nurses to cover our service areas in Rockland, Orange and Dutchess Counties. As UAS Assessment Nurse, you will use your nursing talents to assess pre-screened community members for eligibility in our Managed Long Term Care plan. You will also apply your desire to teach by explaining our services to the potential member and their family. Minimum Qualifications:

  • RN with 2-4 years’ professional nursing experience, including both clinical and functional assessments
  • Previous experience in a home care/community based setting
  • Current NYS RN License and registration
  • Valid NYS Driver’s license and reliable transportation
  • BSN and MLTC experience preferred
  • Gerontology, wound care, and care/case management certification a plus
