Care Manager

Calling all resourceful, collaborative, go-getting Care Managers! EverCare is currently seeking several Care Managers to cover our service areas in Rockland, Orange and Dutchess Counties. As Care Manager, you will play a key role in planning and managing the care received by our Members to help them stay as healthy as possible in their homes for as long as possible. With compassion by your side, you will ensure that each member’s care plan is individualized and specific to their needs, while ensuring that utilization reflects industry standards. We currently offer full-time positions with remote work options, though you must be available to visit members in their homes as needed.

Minimum Qualifications:

Associate’s degree and 5 years of experience in professional nursing, social work, or related; bachelor’s preferred
If RN, must have NYS license and certification
Previous long term care, home care, or managed care experience a plus
Valid NYS Driver’s license and reliable transportation
Bilingual English/Spanish a plus

A passion for the art and science of care planning for optimal results and the desire to empower those in your care to be actively engaged in their health
Strong self-management; you will need to independently balance the needs of your entire caseload, including interfacing with members, families and providers, keeping up-to-date on your monthly calls, and paying a visit as needed
The relentless drive to have an impact and get things done
The natural ability to nurture strong relationships through compassion, communication and collaboration
The understanding of a need for meticulous documentation and effective communication with other care team members
A love of serving senior, those with disabilities, and the vulnerable
